CONSENT FOR MEDICAL TREATMENT
In the event of an emergency where medical treatment is required, I authorize Ridgeway Baptist Church to seek medical treatment and make decisions regarding medical care for my child. I agree to pay for any medical expenses incurred. I understand that Ridgeway Baptist Church does not provide medical insurance coverage for participants and that any costs incurred for medical treatment will be my responsibility.

ACKNOWLEDGEMENT OF RISK
I, the undersigned, as the parent or legal guardian of the above-named minor, hereby grant permission for my child to participate in church-related activities and events organized by Ridgeway Baptist Church. I acknowledge and understand that my child's participation in these activities may involve certain risks, including, but not limited to, physical injury, illness, or other potential hazards that may occur during or as a result of participation.

RELEASE OF LIABILITY
By signing this form, I acknowledge that I have voluntarily allowed my child to participate in these activities, and I agree to assume all risks associated with my child's participation. I hereby release, discharge, and hold harmless [Church Name], its pastors, employees, volunteers, agents, representatives, and affiliated entities (collectively referred to as "the Church") from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by my child while participating in or traveling to/from these activities.

PHOTO RELEASE
By signing below, I also give permission for my child’s photo, video, or likeness to be used by the Church in any promotional or informational materials (e.g., website, social media, newsletters) without compensation or further consent.

IDEMNIFICATION
I agree to indemnify and hold the Church harmless from any and all claims, suits, losses, damages, liabilities, costs, and expenses, including attorney’s fees, that arise from my child’s participation in the Church’s activities or any negligent or intentional acts by my child.

LEGAL CAPACITY AND CONSENT
I certify that I am the parent or legal guardian of the above-named minor, and I have the legal capacity to sign this release. I have read this agreement, fully understand its terms, and sign it voluntarily. I agree that this waiver is intended to be as broad and inclusive as is permitted by the laws of the State of Tennessee, and that if any portion is held invalid, the remainder will continue in full legal force and effect.
